How to Guide
Late For Your Appointment
Please attend your appointment on time, if you are late you may not be seen. If you are not seen you will not be able to rearrange your appointment until the next working day-except in the event of an medical emergency that requires immediate attention.
Alternative Options
Local Pharmacy - As qualified healthcare professionals, they can offer clinical advice and over-the-counter medicines for a range of minor illnesses, such as coughs, colds, sore throats, tummy trouble and aches and pains.
If symptoms suggest it's something more serious, pharmacists have the right training to make sure you get the help you need. For example they will tell you if you need to see a GP, nurse or other healthcare professional.

Minor Injuries Unit at Horsham Hospital - providing assessment and treatment of a wide range of minor injuries (minor head and eye injuries, broken bones, sprains and strains, minor burns and scalds, bites and stings)

Online Appointments
NHS App
We are currently testing the new NHS APP which you can use to manage your appointments, repeat prescriptions and see your health record including test results. It can only be used if you have registered for online services. You can also access 111 online, Symptom checker, data opt out and set organ donation preferences. To register for online services
ALL patients aged 15 years and over will be required to attend Holbrook Surgery in person with photo ID (preferably a passport or driving license).
You will then be given a registration form to complete, or alternatively download the relevant form and bring it with you to the surgery.
Children
Children vary in the age at which they are able to make an independent and informed decision about who should have access to their on-line records. New guidance is regularly being published with varying ages when parental access should be limited. Unfortunately, at the moment guidance is very inconsistent and suggests limiting access from the age of 11 to 16 years old.
At Holbrook we ask any patients over the age of 15 to register for online service in their own right, but due to this recent guidance we need to consider parental access for 11 – 15 year olds. We realise that 11 is very young, therefore we will be routinely asking 13yr olds consent for their parents to access there online services. We may have to change this age to 11 if the guidance changes.

We will then process your application and email you with details on how to activate your account; this will include your patient ID and registration token.
Once registered you will then have access to our online appointments and prescription service through the NHS APP.

Out of Hours
If you require medical advice outside of the surgery’s opening hours, please contact 111.
Out-of-hours services are generally busy so please think carefully before asking to see a doctor and only do so if you genuinely cannot wait until the surgery re-opens.
Minor Injuries Unit (MIU)
Horsham (MIU) and East Grinstead MIU at Queen Victoria Hospital (walk- in) can diagnose and treat a wide range of minor injuries and ailments for both adults and children over one year old; such as: Minor head injuries with no loss of consciousness; Minor burns and scalds; Limb injuries; Simple eye infections i.e. conjunctivitis plus foreign bodies/abrasions; Cuts and grazes; Bites and stings; Ear and throat infections; Skin infections

Treat most injuries or illnesses that are urgent but not life threatening. For example sprains and strains, broken bones, minor burns and scalds, minor head and eye injuries, bites and stings. You do not need to book an appointment – just turn up and you will be seen by either a doctor or nurse.
In a genuine emergency you should call 999. Chest pains and / or shortness of breath constitute an emergency.
Enhanced Access
As part of the government drive to enable patients to access GP appointments from 8am - 8pm, 7 days a week, we offer an Enhanced Access service through our Primary Care Network (PCN) for our patients. The service offers remote telephone and face-to-face appointments with a variety of clinicians, including GPs, Nurses and Pharmacists and are available throughout the week and Saturdays.
The appointments are pre-bookable only and appointments can be made via the surgery.
To access these appointments you must be willing to consent to the sharing of your medical record.
111 Online
111 online is a fast and convenient alternative to the 111 phone service and provides an option for people who want to access 111 digitally.
Your needs will be assessed and you will be given advice about whether you need:
- Treat yourself at home
- Go to a Primary Care Centre
If you need face to face medical attention you may be asked to attend a Primary Care Centre.

Text Reminder Service
We have a texting service which allows you to receive confirmation and reminders about your appointments.
To have this service you will need to register by completing a consent form.
Please remember to update your contact details with us when you change address, telephone numbers and email address.
Interpreter
If you require an interpreter to attend with you when you see your Doctor please notify the receptionist and she will arrange this for you.
Disability & Accessibiltiy
If you have a disability, physical or mental we are happy to discuss arrangements for reasonable adjustments to help facilitate your use of our services.

GP Training & Video Recording of Consultations
The practice has been accredited since 1998 as being suitable for the training of doctors who intend to become general practitioners. This means that the practice & doctors have had to achieve a number of standards which have been assessed by the GP Deanery of Kent, Surrey & Sussex.
We also have post-graduate doctors sitting in with the GPs as part of their foundation training.
For doctors ongoing professional development & part of GP Registrar training some consultations may be videoed (no intimate examination will be videoed). You'll be asked if you would mind co-operating with these essential programmes & you will need to sign a consent form. The receptionists will let you know which surgeries are involved & if you do not wish to participate please let them know.
